In fact, if we live long enough, almost all men will experience some degree of prostate enlargement.</p><p>You might have noticed you are waking up several times a night to pee, or perhaps the flow isn't as strong as it used to be. You may even find yourself hesitating before the stream starts. These symptoms can be deeply frustrating and genuinely exhausting when they interrupt your sleep night after night.</p><p>The good news is that BPH is highly treatable. You do not have to simply accept this as a mandatory part of aging. We have a wide range of highly effective medications that can shrink the prostate, relax the bladder, and help you get your sleep, confidence, and quality of life back.</p>

We are fortunate to have several highly effective medications for BPH. They generally fall into three categories. The first are alpha-blockers, which relax the muscles around the prostate and bladder neck to quickly improve urine flow.

In this group, I frequently prescribe Tamsulosin (also available as Flomaxtra XL ) and Vesomni . Other effective alpha-blockers include Doxazosin (often branded as Cardura ).

The second category are 5-alpha reductase inhibitors, which actually shrink the prostate over time by blocking the hormone that causes it to grow. Finasteride (commonly known as Proscar or Finasteride Proscar ) is a staple here.

You might also recognize the brand Propecia , which uses Finasteride for hair loss, though at a different dose. Another powerful shrinker is Dutasteride (brand name Avodart ).

For men with large prostates and severe symptoms, I often recommend a combination of both an alpha-blocker and a shrinker, such as Combodart , to tackle the issue from both angles.

Interestingly, Cialis (tadalafil), which is famous for treating erectile dysfunction, is also licensed for BPH. It relaxes the smooth muscle in the prostate and bladder. I love recommending this for men who happen to struggle with both BPH and erectile issues, as it treats two conditions with a single daily pill.

What to expect from treatment

Setting realistic expectations is vital. If I prescribe an alpha-blocker like Tamsulosin, you will likely notice a stronger flow within just a few days. However, if you are taking a shrinker like Finasteride or Dutasteride, you must be patient—it can take up to six months to see the full benefit.

A clinical pearl I always share: alpha-blockers can cause retrograde ejaculation , where semen goes backward into the bladder instead of out of the penis. It is completely harmless, but it can be quite a shock if you aren't warned!

Furthermore, if you are ever scheduled for cataract surgery, you must tell your eye surgeon that you take Tamsulosin. It can cause a complication known as "Floppy Iris Syndrome" during the operation.

Self-care and prevention

Medication is only half the battle. Simple lifestyle tweaks make a massive difference. Reduce your caffeine and alcohol intake, especially after 6 PM, as both irritate the bladder and act as diuretics.

Avoiding constipation is also key; a full bowel presses directly against the bladder and prostate, worsening your symptoms.

Here is my favorite practical tip for BPH: beware of over-the-counter cold and flu remedies. Many contain decongestants (like pseudoephedrine) that tighten the smooth muscles in the prostate.

I see a spike in men coming to the clinic with sudden urinary retention every winter simply because they took a cold powder. Always check the labels, and practice "double voiding" before bed—pee once, brush your teeth, and try to pee one more time to ensure the bladder is as empty as possible.
