Treatment depends entirely on your specific deficit. For basic replenishment, I often start with high-dose Vitamin D. Pro D3 , Fultium D3 , and Thorens are excellent prescription-strength options for isolated Vitamin D deficiency, which is incredibly common in the gloomy UK winters.

If you require calcium as well, a combination chewable tablet like Calcichew-D3 is my usual go-to.

For menopausal women, dropping oestrogen levels directly drive bone loss. In these cases, Hormone Replacement Therapy (HRT) such as Kliofem or Indivina can be wonderfully protective while also managing hot flushes.

When bone density has dropped significantly into the osteoporosis range, we bring in a class of drugs called bisphosphonates . Actonel (risedronate) is one I frequently prescribe to slow down the cells that break down bone.

A vital clinical pearl I tell my patients: always take Actonel with a full glass of plain tap water. Never use bottled mineral water, as the heavy calcium content in hard mineral water will bind to the drug in your stomach and render it completely useless.

What to expect from treatment

Bone turnover is a glacially slow process. You will not "feel" your bones getting stronger, and I usually will not order a repeat DEXA scan for at least two to three years because the changes take that long to register on imaging.

However, there is a silver lining. With Vitamin D supplements, patients often report a surprising, noticeable lift in their baseline energy levels and a reduction in deep muscle aches within just 4 to 6 weeks of starting treatment.

Self-care and prevention

Medication is only half the battle. Your bones need mechanical stress to grow. Many of my patients proudly tell me they swim or cycle for hours, but unfortunately, these are absolutely useless for building bone density because the water or the bike supports your body weight.

I always prescribe "stomping." Literally walking with a heavy, deliberate heel strike sends the necessary mechanical shockwaves up your legs to stimulate bone-building cells called osteoblasts . Combine this with balance exercises to prevent the falls that cause fractures in the first place.
